Of course some design elements add to the cost. Height of ceilings, type of roofing, type of foundation, etc. Quality does take more time and cost more money.
It costs more to sheathe the entire house with plywood or osb than it does to use fiberboard (that soft black stuff) and it makes a better house. Rigid steel ducts cost more than flex ducts and are far more durable. Better windows and doors cost more.
The real costs are in finishes. Stain grade trim costs about 6 times paint grade. The range in finished flooring (my cost) goes from about $
